Heart health is the third most common reason people in Avelor hold a supplement: 19 of 72 people with
a saved stack, across 20 saved items. It is also the area where the research has moved most in recent
years, and where the honest summary is less encouraging than it was a decade ago.
**Omega-3 is the most studied and the most argued about.** Large randomised trials and meta-analyses
have examined cardiovascular outcomes (PMID 41201837, 33190147, 36501174), and the results do not all
point the same way. The trial comparing high-dose omega-3 against corn oil (PMID 33190147) is the one
that changed a lot of minds, because the comparator mattered. This guide does not tell you the
question is settled, because it is not.
**There is a documented harm signal as well as a benefit signal, and both are in the same literature.**
Meta-analyses report increased atrial fibrillation with long-term marine omega-3 supplementation
(PMID 34612056, 36103100). Anyone reading only the benefit half of this evidence is reading half of it.
**CoQ10 has better evidence in heart failure specifically than in general heart health.** A randomised
trial examined morbidity and mortality in chronic heart failure (PMID 25282031), and later reviews and
meta-analyses have assessed it (PMID 35608922, 39462324). That is a specific diagnosed condition
managed by a cardiologist, not a general wellness use. Separately, CoQ10 has been examined for
statin-associated muscle symptoms (PMID 30371340, 41158831), where the reviews are more mixed than
the marketing.
Steps
1. **If you have a diagnosed cardiac condition, this is a conversation with your cardiologist**, not
a supplement decision. That is the step, not a preamble to one.
2. **If you are considering omega-3, read both halves of its evidence**: the cardiovascular outcome
trials (PMID 41201837, 33190147) and the atrial-fibrillation signal (PMID 34612056).
3. **Tell whoever prescribes your medicines**, particularly if you take an anticoagulant or a statin.
Duration
The cardiovascular outcome trials ran for **years**, not weeks (PMID 33190147, 41201837). The heart
failure trial reported over a **two-year** follow-up (PMID 25282031). Nothing in this guide has a
short-term endpoint, which is itself worth knowing: the outcomes that matter here are not things you
can feel changing.
Limitations
- **The omega-3 evidence genuinely conflicts.** Different trials used different preparations, doses
and comparators, and reached different conclusions. Presenting it as settled in either direction
would misrepresent it.
- **The atrial fibrillation signal is a real finding, not a caveat.** It appears in meta-analyses
(PMID 34612056, 36103100) and belongs in the same sentence as any benefit claim.
- **CoQ10's heart-failure evidence is in diagnosed heart failure.** Extending it to general
cardiovascular wellness goes beyond the population studied.
- **Red yeast rice contains monacolin K, which is chemically a statin** (PMID 33538260, 31687098).
It is not a gentle alternative to one, and it is not dosed or standardised like one.
- **No dose appears in this guide.** Dosing is a clinical decision and is held for clinician review.
- **This guide has not been reviewed by a clinician.** `reviewStatus` above says so.
Cautions
- **Omega-3 and warfarin: the evidence disagrees and you should know that it does.** There is a case
report describing a serious bleeding outcome (PMID 28033135) and a pharmacokinetic study reporting
no effect (PMID 28197979). Raise it with **the clinician who manages your anticoagulation** rather
than deciding from either paper alone.
- **Red yeast rice with a prescribed statin is additive**, because the active compound is a statin
(PMID 33538260). Raise it with **the prescriber who manages your statin**.
- **CoQ10 for statin muscle symptoms is not a reason to stop a statin.** Muscle symptoms on a statin
are a conversation with **the prescribing clinician**, who has options that do not involve stopping
treatment.
- **New or worsening palpitations** should go to **a doctor**, particularly for anyone taking
long-term omega-3, given the atrial fibrillation signal above.

**Named and not linkable.** **CoQ10** has no canonical entity, despite being one of the two compounds
this guide is mostly about and appearing in live user stacks. **Red yeast rice** likewise has none,
which matters more than usual because it is chemically a statin and is the entry most in need of

**Resolving is not the same as being checkable**, and this is the only guide so far where the
